Associate Course Application

I am voluntarily submitting this “Associate Stage 1 Application” on my own free will and without force.   I understand that this information will be reviewed by the members of the FMM Certificate Board and, possibly, the Board of Directors of Forgiven Much Ministries.  The information I am submitting is true and correct to the best of my knowledge. 

I agree that I am voluntarily submitting this application and that if my application is not accepted that I will in no way hold Forgiven Much Ministries, its board members, personnel, authors, or committee members liable in any way. 

I understand that FMM offers this Associate Study Certificate at no cost to students, however donations to help offset online training expenses are welcomed, greatly appreciated and tax deductible. 

If any of this information changes I agree to notify FMM within 14 days. 

Associate Stage 2 Application

I agree that after completing the ASC Course with 80% comprehension or higher and before receiving the ASC Certificate of Completion, I will complete the “Associate Stage 2 Application” at the end of the ASC Course to confirm or make modifications to my contact information as well as convey any known goals and areas of interest for applying the ASC knowledge in service.
